Presentation of infertility:

  • 1. Sperm abnormality
  • 2. Leydig cell dysfunction

Causes:

  • 1.  Preoperative in association with TC
  • 2.  With Radiotherapy: Rarely permanent
  • 3. With Chemotherapy: Dose comulative dependent

Pre-op measures:

  • 1. Hormonal assay
  • 2. Semen analysis
  • 3. Cryopreservation

Cryopreservation:

1. Most cost effective in all assisted reproductive techniques
2. Preferred to be before orchiectomy.
3. Before chemotherapy and radiotherapy

Post-operative

  • 1. Testosterone replacement. Indicated at:

a. Bilateral orchiectomy

b. Low testosterone after radiotherapy

  • 2. Testicular prothesis
